Put a form on your website
A customer finds your request form from your own store pages, without you asking anyone to edit the site.

Publish the form, then copy the address shown next to it. A form that is still a draft has no address yet.

Open the store page you want it on, or add a new one. Pages you add can show in the store header.

Add a line in the form “Rent an instrument: https://…”, pasting the address after the colon. A line written that way becomes a button instead of text.

It has to be the full address, starting with https://. A partial one stays plain text rather than publishing a broken link on a live shop.

Save, then open the page on your live store and click the button, the way a customer would.
Worth knowing
- The page is yours and carries your branding. The form itself still opens on a bsns address — customers see it, and it is the same form either way.
- A submission creates a customer and a task. It never opens a contract or commits stock, so nothing starts billing because somebody filled in a form.
